Oxford Health Plan

Eligibility

You must be a member of an affiliated association for a minimum of 60 days prior to your effective date of coverage.

You must be self-employed in the arts and entertainment industry and work a minimum of 20 hours per week. Proof of self-employment is required. You must provide tax evidence that you have worked as a freelancer for at least 60 days.

Oxford Health Plans

These Point-of-Service (POS) plans allow members to go to any doctor in or out of the Oxford network. The expenses incurred outside the network are subject to co-insurance of 70%/30% after an annual deductible which varies with each plan. If the member stays within the Oxford network, there are co-payments for office visits and hospital admissions.

New from Oxford for 2008 is a PPO (no referrals necessary) High Deductible HSA eligible (HSA HD Plan) with both in and out of network coverage.
