Oxford Health Plan

Eligibility

You must be a member of an affiliated association for a minimum
of 60 days prior to your effective date of coverage.

You must be self-employed in the arts and entertainment industry and work a minimum of 20 hours per week. Proof of self-employment is required. You must provide tax evidence
that you have worked as a freelancer for at least 60 days.

Oxford Health Plans

We have 5 options through Oxford, including 3 Point of Service (POS) plans that allow members to go to any doctor in or out of the Oxford network, a high deductible HSA-eligible plan, and an EPO plan limited to in-network coverage only.

Note that for all Oxford plans, when outside of the Oxford coverage area of metro-NY, NJ and CT, all United HealthCare Choice Plus providers are considered in-network.
