Health Insurance New York:
GHI (Group Health Incorporated)
TEIGIT offers three GHI plans to Sole Proprietors and
Small Business Owners throughout New York State:
- High Deductible Health Plans (HDHP)
The Small Business Advantage Plan is a PPO, where you
may choose your own providers. If they are in network, you pay
a copay; out of network, you are responsible for a deductible, and then
will be reimbursed a percentage of your medical costs.
The High Deductible Health Plans are much less expensive
options, but high deductibles apply both in and out of network. However,
once your in-network deductible is met, your in-network costs are covered
at 100%. Out of network, after the deductible, you are covered
at 80% of usual and customary charges. The EPO is an in-network
only plan
Eligibility: You must be under
age 65, and self-employed for at least 60 days, working a minimum of
20 hours per week. Two of the following tax documents must be submitted
as proof of self-employment.
- Schedule C (pages 1 and 2)
- Schedule SE
- Schedule K-1
- Schedule 1120-S
- Schedule E
- Schedule F
- Quarterly Wage and Withholding Form NYS-45
A Letter of Certification may be substituted if you
are too new to have applicable tax documentation.
