Golden Rule
Golden Rule offers individual insurance in all areas of Florida. Unlike the Group Insurance offered by CIGNA, Oxford or GHI, this insurance is medically underwritten and is not guaranteed issue. Rates vary according to age and health status. Your dependents may be covered under your individual policy.

CIGNA
CIGNA offers a PPO plan in selected counties in Florida (see below). You must be under 65, a member of your association for a minimum of 30 days prior to your effective date of coverage and self-employed in the arts and entertainment industry

| If you would like to apply for CIGNA coverage, print and complete the Rate & Application Instructions and Enrollment Form and return to TEIGIT with the appropriate payment. Applications must be received by the 20th day of the month preceding the effective date. | |
