Cigna Dental PPO Plan

Eligibility

You must be under age 65, a member of your association for a minimum of 30 days prior to your effective date of coverage and employed in the arts and entertainment industry.

Enrollment Dates

New enrollments for the CIGNA Dental PPO are accepted on a monthly basis. Applications must be received prior to the effective date.

The CIGNA Dental PPO Plan provides both in-network and out-of-network coverage utilizing the larger CIGNA Radius Network

The CIGNA PPO Plan is not available in New Jersey

If you would like to apply for coverage, print and complete the Information and Rate Sheet and Enrollment Form and return to TEIGIT with the appropriate payment. Applications must be postmarked no later than your effective date.
